Abstract
The expansion of home-based anticoagulation therapy has necessitated the development of robust regulatory compliance frameworks to ensure patient safety and therapeutic efficacy. This observational cohort study evaluates adherence to regulatory guidelines among patients receiving anticoagulant therapy in homecare settings across multiple regions. Data were collected from 240 patients over a six-month period, focusing on prescription accuracy, monitoring frequency, and adverse event reporting. Findings indicate that structured pharmacist-led interventions significantly improved compliance with regulatory standards, particularly in dose adjustments and INR monitoring. However, inconsistencies were observed in documentation practices and patient education, highlighting gaps in regulatory enforcement. The study underscores the importance of integrating digital health tools to enhance compliance tracking and reporting. Furthermore, interdisciplinary collaboration between pharmacists, physicians, and homecare providers was identified as a key determinant of successful outcomes. These results support the implementation of standardized regulatory protocols and continuous professional training to optimize home-based anticoagulation management. Strengthening Medication Safety in Homecare Pharmacy Through Protocol-Driven Practice Models

Abstract
Medication safety in homecare pharmacy services presents unique challenges due to decentralized care environments and varying levels of patient supervision. This cross-sectional study investigates the implementation and effectiveness of medication safety protocols among homecare pharmacy providers. A total of 180 homecare cases were analyzed, focusing on dispensing accuracy, labeling compliance, and patient counseling practices. The results reveal that while most providers adhered to basic safety protocols, significant variability existed in advanced safety measures such as electronic verification systems and follow-up monitoring. Errors were predominantly associated with polypharmacy and inadequate patient education. The study highlights the critical role of standardized safety checklists and pharmacist-led interventions in reducing medication-related risks. Additionally, regulatory gaps were identified in the enforcement of safety protocols across different jurisdictions. The findings suggest that integrating technology-driven solutions and enhancing regulatory audits can significantly improve medication safety outcomes. Regulatory Influences on Telepharmacy Practice in Home-Based Chronic Disease Care

Abstract
Telepharmacy has emerged as a transformative approach in delivering pharmaceutical care to patients with chronic diseases in homecare settings. This mixed-methods study examines the impact of telepharmacy regulations on service quality, patient adherence, and clinical outcomes. Quantitative data from 210 patients with chronic conditions were complemented by qualitative interviews with healthcare providers. The findings demonstrate that adherence to telepharmacy regulations significantly improved medication adherence rates and reduced hospital readmissions. However, regulatory inconsistencies across regions posed challenges in service delivery, particularly concerning data privacy and cross-border consultations. Patients reported high satisfaction levels due to increased accessibility and timely interventions. The study also identified technological barriers, including limited digital literacy among elderly patients. The integration of standardized telepharmacy guidelines and training programs was found to be essential for optimizing outcomes. Regulatory Challenges in the Distribution of Controlled Substances for Homecare Patients

Abstract
Controlled substance administration in home care facilities creates a complicated regulatory issue because close surveillance and access to patients are required. This qualitative policy analysis understands the current policy regulating the distribution of controlled drugs in various countries. The policy document reviews and expert interviews with regulatory authorities and healthcare professionals were used to collect data. It is found that there are huge differences in the methods of regulation especially the section of prescription validation, storage conditions and delivery processes. Although strict laws have been enacted to curb abuse and diversion, they tend to create problems to patients in need of promptly accessing the necessary medicines. The research recognizes the most effective practices as secure electronic prescribing systems and pharmacist-led verification process as the effective measures to balance between control and accessibility. Also the contribution of homecare providers to compliance with storage and administration practices are highlighted. The results indicate that regulatory harmonization and technologies-based solutions could reduce risks and enhance the accessibility of patients. The research will help to develop more effective and patient-centered controlled substance management regulatory policies in the homecare setting.

Benchmarking Homecare Pharmacy Accreditation Practices Across High Income Healthcare Systems

Abstract
Standards of accreditation are paramount to the quality and safety of homecare pharmacy services. This comparative analysis paper will assess the adherence to accreditation standards among homecare pharmacy providers in the developed nations. Information was gathered using 75 accredited facilities which included infrastructure, staff qualification, documentation practices and patient care protocols. The findings show that compliance levels were high in personnel training and medication administration, and low levels were observed in documentation consistency and performance evaluation measures. Dissimilarity between the accreditation standards of the countries helped create discrepancies in the quality of services. The research places emphasis on the need to have continuous quality improvement programs and regular audits in order to ensure high level of care. Moreover, the introduction of digital documentation systems was another reason that led to the improvement of compliance and operations efficiency. The results highlight the importance of having cross border cooperation to formulate standardized accreditation systems that can attract uniform quality in homecare pharmacy services. The study makes significant contributions to regulatory authorities and accreditation agencies that would enhance the quality of healthcare delivery.
